I was a surgical resident at Johns Hopkins, training to become a cancer surgeon. I believed I was learning the highest form of medicine: the dramatic, last-minute intervention that pulls a patient back from the brink. I was training to be a hero in the emergency room and the operating theater. But slowly, a deeply unsettling realization took hold. I was not saving lives in any meaningful sense. I was arriving too late. A patient would come in with a massive tumor. We would operate, sometimes successfully, sometimes not. Even when the surgery was technically perfect, the patient often faced a diminished life, weakened by the disease and the treatment. I began to wonder: what if we had caught this ten years earlier? What if we had intervened before the cancer ever had a chance to take hold? The current medical system, I saw, is not designed for that. It is a system of delayed action, waiting for disease to manifest before deploying its most aggressive tools.
